How to Register

​

• Click Register Now button 

• Submit Completed Form 

• Within 24 hours you will receive an email confirming your registration and requesting additional information.

  • If you are a TRICARE beneficiary, your class is covered 100% and you will be sent a form to share your information to ensure your class is covered. A Credit Card Authorization Form will be requested to hold your seat. The card will NOT be charged, except for in situations such as no-shows without notice (all stipulations outlined in registration documents).  

  • If you are self-pay, you will be sent an invoice to pay for your seat in the class.Payment is required to reserve your seat. 

  • After these forms/invoice are returned you will be sent an email with information about attending your class including location (if in person), access link (if online), and other important details. ​​

• If you can not attend the class unexpectedly, you will be automatically enrolled in the ONLINE class for that day, and sent the replay if you can not attend LIVE. 

• If your class is cancelled or rescheduled, Jaimie will contact you ASAP via email and text message. You will have the option for a refund or to apply your enrollment fee toward a future class / online class

• If you are self-pay, you may request a Superbill to submit to your insurance company for possible reimbursement. Jaimie Zaki can not guarantee reimbursement.

Location: Currently classes are being hosted in a private residence, therefore location details will be provided after registration is complete. Location is approximately 5 minutes from Sheppard Air Force Base.
