Is Your Baby Having Trouble Latching? How to Overcome Tongue Tie Challenges for Successful Breastfeeding

Tongue tie, or ankyloglossia, is a condition present at birth where the tissue connecting the bottom of the tongue to the floor of the mouth (the frenulum) is shorter or thicker than usual. This limitation can limit the tongue's movement and your baby’s ability to latch onto the breast effectively.


Tongue tie can vary in severity. Some babies may not have obvious issues, but others might struggle to breastfeed properly, leading to added stress for both parent and child.

Identifying the signs of tongue tie early on is key to resolving any potential feeding issues. Watch for these common indicators that may suggest your baby has difficulties latching due to tongue tie:


  • Gumming or sucking without effective milk transfer

  • Producing a clicking sound during nursing

  • Not being able to extend their tongue past the gums

  • Experiencing a shallow latch or causing nipple pain for the mother

  • Showing poor weight gain—about 10-12% weight loss in the first week post-birth is concerning, or not gaining on a growth curve


If you see any of these signs, consulting with a lactation consultant for a detailed feeding evaluation is a wise step.


Understanding the Impact of Tongue Tie on Breastfeeding


Tongue tie can lead to multiple breastfeeding challenges that may discourage mothers. Here are some common problems:


  • Inadequate Milk Intake: A baby with tongue tie often struggles to create the suction required for effective milk extraction, which can lead to insufficient intake and dissatisfaction during feeding.

  • Nipple Pain: Mothers may experience sore or cracked nipples due to improper latching. Approximately 70% of breastfeeding mothers report nipple pain, which can impact their decision to continue nursing.


  • Milk Supply Issues: If your baby is not adequately removing milk, your supply may decrease. Studies show that insufficient supply is a leading cause of early breastfeeding cessation.

Consult a Lactation Specialist


Connecting with a lactation consultant is one of the most effective steps you can take. We can evaluate your baby’s latch, oral function, and provide tailored advice to improve feeding, even if tongue tie is present. They may also offer comfort measures, positioning tips, and specialized tools to facilitate easier breastfeeding.


Tongue Tie Release


If your Lactation Consultant identifies oral dysfunction consistent with oral ties, she may refer you to a dentist or ENT who performs a procedure called a "frenotomy", which surgically releases the tight tissue. This is a quick and simple procedure, however should only happen when the baby is ready for release. A release without readiness can have negative outcomes making feeding harder. Ensuring your IBCLC and release provider are communicating closely is important.


Implement Different Feeding Positions


Trying various nursing positions can help your baby latch better despite tongue tie. Positions like the side-lying or Koala hold may offer you more control over the feeding angle. This adjustment can ease discomfort and improve your baby's ability to suckle effectively.
